To place a single account for collection please access the appropriate form below. For multiple accounts, you may email your account information and attachments or you may fax this information to 847-934-0776. For accounts placed online, you will receive an email acknowledgment confirming receipt of the accounts. All other accounts placed by fax or phone will be confirmed by a letter of acknowledgment.
